Smooth Transitions MN LLC is a provider of Housing Stabilization Services (HSS) in the state of Minnesota. This Medical Assistance (MA) benefit allows us to help people with disabilities and seniors navigate the many challenges they face when finding and keeping integrated housing in their community.
